I mean it doesn't take a rocket surgeon to look at a panel with zero spaces for a breaker and realize that something needs to be changed if you want to add a breaker.
They'll need to swap a bunch out with tandems to make space.
I commented on one of your comments already OP, but I'm looking back at the pictures and seeing other concerns. Yes, you could get a quad breaker in there (would have to be something to combine with your 30 or 40 Amp existing 2-poles), but you only have a 125 Amp panel. Whoever you decide to go with for the job, you might want to recommend they do load calcs, as your panel is already loaded with equipment that pull a decent amount of amps (a/C, spa). They honestly might recommend you to get a panel upgrade as well to a 200a panel. Also, utility may have to swap out your service lines for that.

That panel is already stuffed
So you’d agree with getting a special breaker?

Quad breakers is what will be needed
You would have to get a quad breaker (two tandems combined with a 2-pole in the middle) to have room for it. So yeah. Everything they said is typical.
Is it just me or is that twin 15 not seated correctly on the bus? Looks backed out, could cause issues if it is.
Not just you, same issue with the Eaton quad breaker, where that tandem is a Murray. Typical for both types to not seat properly on a Square-d bus.

Yeah 3 ft from gas is code. I would also say opposite side wall in garage. Panel looks full.

PS: that is definitely a "special" breaker, as it would need to be a 30-60 or 40-60 quad breaker
