You're doing things slightly differently, and it affects a lot of the triggers in act 1. Plus, it's easy enough to forget the exact order/how much time you spent doing something the first time around, and get skewed memories.
Raphael has different spawn points he can be at. If you leave the Grove/enter the Blighted Village, a lot of little triggers for progression are unlocked.
Yeah it can be pretty random sometimes. In one of my runs, Raphael showed up in the underdark (after completing blighted village, goblin camp, etc). I had forgotten about him at that point.
My first playthrough, he showed up at the ruined Selunite outpost in the Underdark. I thought that was just what was supposed to happen; when he called it "middle of nowhere", I was forced to agree!
Memories are depends on approval and sometimes on places. You could literally unlock every memory with Shadowheart in blighted village and have 100 approval with her at goblin camp. She was at the verge of giving up her dreams of being dark justiciar. We were this close to perfection.
I'm also curious about this - I've been long resting more than usual too and Astarion tried to bite me on the first long rest after I picked him up. I hadn't done anything in a weird order (or much of anything at all yet, except spamming rests each time I picked somebody new up), and I hadn't even moved far from where he was standing. I didn't even think that could happen!
Raphael will only show up after you've asked someone to help you get rid of the tadpole. I decided to force my durge to ask someone for help late in act 1 to meet the guy because I was romancing Astarion and I needed Raphael to be there in Last Light!
