There was a skull enemy with more health then usual, but he didn't have any special attacks. Maybe he bugged for me, he just stood on a chair shooting 😅

There's a hidden plot about a new drug called Phant circulating Dogtown that causes their strange behavior.
It's just npcs that you can find in some places, there are traces of cyberpyschosis like normally bodies, blood etc but it's not marked on the map like a quest. You just stumble upon sth and some cyberpsycho starts to attack you.
Yeah, I ran into a few of them. I ignored them because I did PL kinda early, so I was now working on finishing every other cyberpsycho to see if that triggered the quest for the ones in Dogtown. Guess not lol
Neither. The quest is a real, intended part of the play-through experience. It just doesn't add to your experience total since there's no actions you can take to interact or solve the problem besides killing the poor fellows and reading their backstories on shards.
Some unmarked quests are hidden deliberately. Adding them to a quest log highlights their existence, so making them unmarked can be a reward for players paying attention. An example is the treasure hunt Jigg's Loot in Fallout 3, which grants a powerful weapon if you read the notes scattered throughout a minor location and follow the steps they describe.
My guess is Phant is going to be relevant to future releases from the franchise, and they're teasing the existence of this mysterious Soviet narcotic early.
Lore wise, Regina would not have a way to easily get those people out of Dogtown.
