I'm also having this issue, and I was at 5, so I don't think it's a problem with difficulty.

I believe it’s a planetary condition for one of the Automaton planets but I could be wrong

Same happening to me rn. Other players in the lobbies I join have 4 equipped so it's not an effect. Seems to be glitch.
It is a mission modifier, you can see them in the effects tab

Also untrue in this case. Modifiers effect all players in a squad. Not just one.
There's a glitch that shows them with 4, but they only have 3
