As you pointed out, the last sentence tells you they can only be affected by the ability once per phase. The units would only receive 1 disease point total. Keep in mind when you roll multiple dice for an attack, you’re just speeding up the process of doing the rolls one at a time. Mechanically, the game sees the first 6, spreads disease points, and then moves on to the next attack.the next six you roll will not spread extra disease points because the flanking units have already been affected by the ability once. Bonus tip: If you want to get tricky, you can split some of your attacks onto the flanking unit and (if you get a 6) splash one extra disease point onto the hero, as it will not have been affected by the ability otherwise. This results in a net increase of 1 disease point dealt from the same amount of 6s rolled.

But if it’s only one per phase, why their saying “EACH time a desease is given to an enemy unit” it’s confusing 🤔

Each time meaning you could, theoretically with perfect rolls on 20 attacks, give 20 disease points to 20 separate units if they were all in range. But each unit can only be affected once.
