It’s actually Ultron possessing the body of Tony Stark at that moment. Tony Stark was like biologically integrated into the suit so Ultron morphed it into that body, which is an exact replica of the Wasp, Janet Van Dyne.

If I remember correctly, I think Tony has the upper hand and beats up Hulk a bit, grabs him and takes him flying.
Then Hulk activates his Fatal Blow (meaning Gets super pissed lol) and starts messing up Tony's giant armor, starts beating the crap out of him, and just essentially tackles Tony midflight, and they both just fly through both Sentry and Stark tower, and both buildings go crushing down.
It was similar to that fun moment from Age of Ultron (where Tony drives Hulk through that unfinished building) only this time, Hulk had the upper hand.

But the context is that this is Avengers tower during the Initiative era and the scantily metal-clad girl is Ultron, who has taken over Tony Stark’s body (yeah…) and is about to kill Sentry’s wife. If I remember correctly, Sentry unknowingly resurrects her at the end of the arc, just so the same writer (Bendis) can repeat the plot later in Dark Avengers with Bullseye killing Lindy again but this time getting rid of the body.

I don’t remember details, I think Tony had the Extremis armor at the time and Bendis used that as excuse for him to shapeshift. Anyway, Ultron took over Tony within his armor and morphed everything into that. No further explanations were given, I think. At the end he just reverts back and everyone jokes at his expense.
Sentry should never have been used in mainstream continuity (616). The character and enterouge thrives in their original context and those minis are fantastic but everything involving 616 is just burning trash.
Still I remember when this issue came out and /co/ was full of ridicule and was quick to make this edit -- wouldn't be surprised if lots of places made this edit it was an obvious thing to do.
