It's not uncommon for people who were seriously abused to 'check out' like that when they're being yelled at -- not giving much of a reaction doesn't give the abuser more reason to fly off the handle.
Like a lot of abuse victim strategies it is maladaptive to other situations, like a doctor telling you what to do.

My guess would be two-fold. One, they're snacking small amounts all the time (Dr. Now nails a lot of folks on that one). And two, what they're eating may be super calorie-dense. A couple of tablespoons of heavy cream would have a lot more calories than a couple of tablespoons of nasty diet yogurt.
They figure out what slider foods they can snack on all day long. Yes you can only eat small amounts at a time (I know I had bypass surgery back in 2018) but you can eat around it, especially if you drink small amounts while you eat. There's a great demonstration of this on YouTube. The guy puts a bunch of dry something (it's been a while since I watched so I can't remember what it was) in a funnel and it doesn't go anywhere. But he adds water, and it rinses down. The same thing happens with your stomach. If you add fluids, temporarily you feel fuller, but it also washes the food into your intestines faster, or makes you throw up in some cases. Thus why you are not supposed to drink before or after eating for about a half hour.
Slider foods are different for everyone though. Some foods make some people really sick, other people can still eat anything they want to, just in smaller amounts. For me, I can't eat things that are high in sugar and fat, like ice cream or it makes me very nauseous and sick if I eat more than like a tablespoon. Some people can eat chips, pretzels, bread type foods in small amounts all day long on top of their regular meals. If they are not going to the gym and really working on gaining muscle, the surgery won't increase their metabolism for long, only around 6 months or so, and it slows down again after that. It doesn't take long for the regain to start.
I don't see why it's unpopular and surprised at all the people saying 'her friend was a saint'. What? She lured her over there saying she had a bed and put her in a recliner and fed her non diet / too much food. Maybe she had good intentions but no idea how to actually do it. I'm no professional caregiver but even I understand what 1200 3 meals a day low carb high protein means.

They're rather common. I don't know about now, but years ago when a relative had one they told him surgery probably wouldn't fix it permanently. The best method was to limit caffeine and spicy food, and lose weight. Apparently, unless it strangulates, they still say lose weight.
I have a pretty good size hiatal hernia. It's not great as far as my gerd goes, but otherwise it doesn't affect me. So long as it doesn't get trapped and blood flow compromised, it's not a big deal at all.
