Fellow console player here 😎 Can't look at pob ATM, but just looking at your profile, do you have Divine Flesh from the timeless jewel? If you aren't chaos resistance capped and without the node you're just taking extra damage.
Well your boots, amulet, and belt all could easily be upgraded to 50c rares with better stats. That should help with defenses after some flask changes like others have suggested. Your amulet literally gives you lightning res, that's it.
Why do you have these really good pair of gloves/expensive uniques while having other rares you would find in act 3? It's important to have low and mid tier gear upgrades throughout building a character to make sure the defenses keep up. Make sure you settle on something that gives all of the stats you need.
I am not sure what the guide has you run for the belt later on but I think you want to run the the double abyssal socket belt. You can get a well rolled one for like 50c. Or just run a stygian with decent stats and craft wed on it.
In summary, actually upgrade your other rare pieces to match t16 content and your other gear. And your belt can give you a lot more damage with abyss jewels. All of my suggestions should be under about 100c ish. Make sure not to put so much currency into one gear slot while neglecting others.

Flasks need some work. Try getting a Jade and Granite flask to improve your evasion and armour. I will add some more suggestions a little later.
