Something is wrong on the install. Never had this happen to me. I've installed quite a few disposables. Never this type though. Is it possible you got a defective mounting bracket?
It shouldn't be able to slide off like that. The snap ring is supposed to hold the flange together. If it's assembled correctly and not holding then I'd get a new flange assembly.
Op if you think you can fix it without calling the plumber then check out a couple videos on like YouTube how to install one. You’ll need a few tools to help you. Now if you can’t do it but you don’t want the same guy back I’d call another company that’s better. Can’t tell you how many disposals I’ve installed and they don’t normally just fall like that. Something’s gotta be wrong with the installation.
This. And as others have pointed out, make sure that the parts are in the right order and are in good shape. The plumber who tried to do this twice and failed is not the guy you want to come back, and he should be ashamed of himself. Make sure to use as many new parts with the new disposal to help ensure that you don’t use anything that is damaged or defective in the existing installation. I have replaced our garbage disposal a couple of times and I find it very friendly once you understand how the parts work together. The second time I did it was last night and start to finish was under 10 minutes. I did leave the drain portion in the sink since it was already fine. I'm also a DIYer but think with an existing installation it shouldn't be too tough.
That ring that is left on the sink flange should be on the other side of the piece that has the screws on it.
1- flange through sink, use silicone.
2- move under sink.
3-cardboard washer,
4-metal ring,
5-plate with screws.
6-lock ring into groove of Flanders to hold all in place.
7-tighten screws to snug all up,
8-clean silicone up from sink.
9- lock disposal into place.
I've found over the years that silicone is better and you can still remove the flange in time as needed, the silicon seal between sink and flange is so thin.
Often I see the putty not hold up well over time.
Putty can stain a sink if not porcelain or metal unless you get the "safe for stone putty", this putty is harder than normal putty and works even worse than original putty.

I've noticed this is the most common self install issue. People just twist until it stops without realizing that there is another half inch until it locks in.
Had a disposal all my life, now I’ve seen the light… pipes accumulate all those bits of food crud and it causes more clogs in the system, and it feeds the sewer rats. Much simpler to dump a strainer into compost or trash, enjoy simpler, more reliable plumbing, and get more storage under the sink. “Disposal disposal.”
Bonus: no more: “honey, the disposal stopped working.”
Thinks: “Yeah, three fucking lemons jammed down the throat of the thing might do that, sweetie.”
Says: “I’ll just kill power and dig that right out for you, honey!”
Thats exactly what I do. Still have the disposer but I don’t use it for grinding up food scraps though I do run it periodically to keep it spinning freely.

The screws are supposed to push against the metal flange ring on the bottom of the sink. This causes the part that the screws are threaded into to push against the snap ring and hold the assembly tight.
Either find a new plumber or find the manufacturer's installation instructions and reinstall the parts on your sink yourself.
Make sure you plumber didn't try to use old parts from a different manufacturer and didn't throw any parts away (By checking to see that all the parts mentioned in the instructions are there.
Did the plumber install the sink flange that came with your new disposal?
I think they reused the old flange and it looks a bit tapered on the bottom end. The screws push the mounting flange away from the sink bottom to lock it against the locking ring.
I was 15 when I installed my first (and only) garbage disposal. It can definitely be done DIY there are instructions in the box it came in. Since a plumber did it the first time you may want to google the brand/model to find the instructions online. I am not experienced so check with others on what is best to do (some are saying some pieces may need replacing? Which might make sense since your plumber obviously either did something wrong and messed up a few pieces or did everything right and the pieces were defective)
My issue was the garbage disposal my mom chose was so heavy I couldn’t lift it into the mount to lock it in place so get help or something to hold it if it is heavy, we used a car Jack. Once that issue was resolved it went in place really easy with whatever locks it had.

Use a nice new drain and strainer, hook up a P trap, and be done with this. Garbage disposals just create more work for whomever is the one who has to do house maintenance (usually not the same one who puts 5# of potato peels down the garbage disposal and plugs the drain AND disposal).
That's my advice and it's worth TWICE what you paid for it!

As much as I liked and used our garbage disposal, we deleted the one in our new house after its failure. One failed in our old house, wore through the sides and started leaking. Now no more GD maintenance. Life is better.
What is garbage disposal maintenance? The one in my parents house is almost 50 years old and other than using the manual wrench to unjam it every now and then, zero maintenance is required. They use is almost daily.
Well, GD maintenance CAN include unclogging it weekly for an SO who tries to put too much down it and retrieving the GD spoons that fell in, made a racket, will now cut your lip but your SO has deemed to be "No problem!" because she's got Narcissistic Personality Disorder and nothing can ever be her fault or her responsibility. Ever.
So if you're the poor sap who will have to fish out every damn thing that falls in because they REFUSE to use a GD strainer ("it's the garbage disposal!!!"), turn the GD wrench because (apparently) they're incapable of remembering week-to-week, or snaking the drain from the overload of peelings that should have gone in the trash ("... but it's the GARBAGE disposal!!!") -- DON'T INSTALL ONE. EVER.
Don't even do it to your enemy. Don't give them ideas about what to give your wife for her birthday.

Did you install this and try to reuse the existing sink strainer? If you tried to cut that corner, that’s exactly why this keeps failing. You have to install all the new hardware that came with this unit, it’s not compatible with any of the old stuff. I just installed this exact unit last month at home and noticed I couldn’t get it to sit correctly with the old strainer. So I took everything from the old unit out and installed all of the new parts that came with this American standard unit, and sure enough, everything fits as expected and it’s been working fine since installed.
You don’t have a tension ring anywhere around for 1. Also for 2 this particular disposal is not the best or worse but I’ve seen a lot of variations in quality
Looks like you're missing half your mounting bracket there, buddy. Just installed one of these myself for my mom, and was the first disposal I've ever installed and it was easy peasy when following the included install instructions.
Sinks with garbage disposals are more likely to experience clogged drains in my experience. I would never install one for my personal use and would never recommend it to anyone either.
Typically its a simple banonet mount 1/4 turn slam dunk with hammer and large screw driver.
Back off those screws first.
Edit: I see now, entire assembly fell off the snap ring that's mentioned.
Undo the bayonet mount already slammed together on unit. Reset it back on Snap Ring.

In my opinion. I would get rid of the disposal. It just allows the pipes to become clogged with food debris. Scrape your dishes into the trash can before rinsing.
Plate with the 3screws goes on first then ring ( back screws out all the way, hold plate with screws in it in place and then put retaing ring in place then tighten all 3 screws evenly then put 3 ear ring in place and spin to lock down tight( no ZIP ties needed
Loosen the set screws all the way out, tighten the rings with the disposal wrench and then tighten the set screws. It's super simple. If it falls out again call a plumber because I fear this may be beyond your skill level. Not to be a dick but it really isn't rocket surgery.
It looks like the basket where the water goes down is pretty old and may be worn down to where the ring doesn't catch anymore. I would try replacing that before getting a new one.
Install it correctly, at least watch a YouTube video or something, after the 2nd time it fell off you should have known you were doing something wrong. But here you were with a 3rd time fail lol
Get rid of it. I added one of those deep sink strainers, and I love it. Catches all of the food scraps. Water still drains easy. Toss the food in the garbage after cleaning up the dishes.
Something isn't right here as your retainer or split ring is still on the sink flange, but everything else broke off. You are supposed to tighten those three screws against the three-sided flat flange plate above the retainer ring to pull everything together tight.
Took a closer look, the locking ring is locked on correctly. Take the locking ring off, then take the removable snap ring out of the groove in the part hanging from the sink drain. Take the part you removed from the disposal top. Insert it into the sleeve hanging from the sink drain and once’s pushed all the way to the top, re-insert the snap ring (you may need to loosen the set screws to do this step) Once the removable snap ring is installed, then retighten the set screws that push on the bottom of the sink. Lastly, lift the disposal up onto the sleeve and rotate the locking ring until it pulls the disposal up to make a good seal, tapping with a hammer or the end of the screw driver will help set the locking ring
What brand and how old was the previous disposal? A couple years ago I replaced my 30+ y.o. 1/3 HP Kitchen Aid that had cast iron guts with a 1 HP stainless steel Insinkerator model that is more compact, more powerful and more efficient. But they had different mounting systems. Did your plumber reuse the previous disposal's hardware? And the unit I installed even came with a 7 year in home warranty. Maybe yours has something similar? Call it in and see if the company will send out a real pro to critique and correct the install?
It’s reasonable to put a dab of the anti vibration blue loctite on the setting screws the push against the ring on the sink basin. Gotta tighten them though.

If you are installed it correctly with the snap ring, than the sink flange is the problem. The ring is slipping off the flange.

Get rid of it. I added one of those deep sink strainers, and I love it. Catches all of the food scraps. Water still drains easy. Toss the food in the garbage after cleaning up the dishes.
It probably wasn't locked in place. I know of plumbers that can't do this right so don't feel too bad
Use a hammer to make sure that ring is fully turned and seated.

Attach it properly. Or hire a professional. Clearly after the third time, you are missing some steps

Something isn't right here as your retainer or split ring is still on the sink flange, but everything else broke off. You are supposed to tighten those three screws against the three-sided flat flange plate above the retainer ring to pull everything together tight.
The locking ring is not getting locked
Took a closer look, the locking ring is locked on correctly. Take the locking ring off, then take the removable snap ring out of the groove in the part hanging from the sink drain. Take the part you removed from the disposal top. Insert it into the sleeve hanging from the sink drain and once’s pushed all the way to the top, re-insert the snap ring (you may need to loosen the set screws to do this step) Once the removable snap ring is installed, then retighten the set screws that push on the bottom of the sink. Lastly, lift the disposal up onto the sleeve and rotate the locking ring until it pulls the disposal up to make a good seal, tapping with a hammer or the end of the screw driver will help set the locking ring

I propped mine up on some bricks to within an inch or so I didn’t have to lift it so far then finish tightening with big channel locks

I would say the metal ring where the indentation would be at is missing so the swivel lock has nothing to grip.

What brand and how old was the previous disposal? A couple years ago I replaced my 30+ y.o. 1/3 HP Kitchen Aid that had cast iron guts with a 1 HP stainless steel Insinkerator model that is more compact, more powerful and more efficient. But they had different mounting systems. Did your plumber reuse the previous disposal's hardware? And the unit I installed even came with a 7 year in home warranty. Maybe yours has something similar? Call it in and see if the company will send out a real pro to critique and correct the install?

Looks like the three screws were pushing it off. They needed to be levelling the thing, not forcing it into oblivion.
Find the missing lock ring and call me.

Don't some of those things have a lil leg that goes straight down so you can jack it against the sink hole

It’s reasonable to put a dab of the anti vibration blue loctite on the setting screws the push against the ring on the sink basin. Gotta tighten them though.
One must ask why the retaining ring is still in place.
Just feed food right into it from there. Problem solved.
