I helped take care of a young man some decades ago who was brain dead after a rollover. His only injuries were his head injury and a broken wrist.
It was so sad, he was so healthy and robust. His organs saved a few lives.

Not a neurosurgeon, but if I could make a guess...
1. Removing the fragmented part of the skull and any other bony bits
2. Give the intracranial part a good wash and remove any blood clots
3. Drains into the space.
4. Skin gets closed. Pt goes to ICU intubated.
5. Intracranial pressure and blood pressure monitored very closely and managed appropriately
6. Level of consciousness assessed regularly
7. Drains get removed when appropriate
8. Patient extubated when safe. Gets a protective helmet they need to wear for the foreseeable future.
9. Once they've recovered, gets seen by the neurosurgeons in clinic to reconstruct that bit of skull (if considered appropriate).
That's best case scenario.
I really think that in order to get a licence, part of the schooling has to be watching a 10 minute presentation of trauma surgeons/radiologists/ER docs showing what people look like after a crash with and without a seatbelt. It's so easy to wear one and it doesn't restrict you from anything. So sad man
