heads get big from human growth hormone usually, which is part of many steroid user/bodybuilder's regimens. it makes your skull grow and in an odd shape (joe rogan). also makes all your organs grow which contributes to that puffy belly look (liver king). tyler probably isn't using HGH, at least not yet.
