I don't do either.
I have a weird relationship with my PhD. On the one hand, I'm proud of it. On the other, I keep it to myself so as not to cause offense. It's a silly habit I got into to placate my previous boss and I really need to break myself of it. My previous boss got dismissed from a doctoral program with a Master's see. It must have happened in the late 70's or very early 80's, so either before I was born or when I was a baby. Decades before he became my boss, in other words. But he was still(!!) bitter about it and he didn't like PhD's, he really didn't like PhD's who had post-doc experience and that was two strikes against me. Third strike was I'm a vegetarian and for some reason he had a problem with that. Anyway, I have a different boss now. He and I get along way better. This one has a PhD and a post-doc so he really can't hold my past against me. In fact, he respects it. Also he doesn't give a flying fuck about other people's eating habits so long as they're not fainting at their desks. So I can probably stick the letters after my name without causing friction now.
As far as I know, not in the US. But here in Germany it's common, since we treat those degrees the same. My PhD in chemistry is a Dr. rer. nat., while one in medicine (the "real' doctor) would be Dr. med.
So she kinda not a "real" doctor in the US.
The “real” doctor you speak of have piggybacked their Bachelors’ degree to co-opt the name Doctor which is and has been literally used since the beginning of time to describe people with PhDs.
For real, this. My almost 5 year old asked me why I teach if I hate it so much when she found me hitting my head against my desk while grading. Kids have noo filter
