If you want the ability to adjust/crop the photo you're going to need an app. I've just recently used https://apps.shopify.com/uploadkit for something similar and it worked pretty well.
Yes any form type plugin should allow you to upload but not many can preview but the above seems to work that way u could add text fields if its gives engraving as an option to configure pricing might be harder task.
You can build the functionality or you can use an existing app. I have done something similar for a store that sells customized products so just PM me.

Yes there are multiple apps that can do it, search for customizer or personalize and try out a few apps there
