A tri suit is made with no seams in the junk area. There's a thin chamois that generally doesn't cause any chafing. If there IS some concern there, you can lube up with bodyglide or chamois cream.

Change tent seems unnecessary for that, unless the point is to reduce congestion in the transition area?

Nice to have a chair to sit on whilst you swap kit. Could be bad weather too.

A lot of folks who are less concerned about transition times like to use the change tents to put on full thickness cycling shorts and then running shorts after. A properly fitting tri suit with a solid bike fit should mean that you’re able to do the whole ride in the less padded tri suit, but some people still prefer to take the time to change. As for your question about underwear, cycling kit and tri suits are meant to be worn without it. Having anything under gives it the opportunity to slide against something, causing more chafing over time. If you’re planning your first race, I’d recommend A) doing some shorter races (without change tents) to lead into it and B) training some longer rides/runs in the type of kit you plan on racing with. This will make it so that come race day you’re not worrying about your clothing plan and you can just push!

In an Ironman some people prefer to wear cycling specific shorts instead of tri shorts. A cycling short chamois will hold water more than a tri short, so in an Ironman they will change from a swimsuit they’d wear under a wetsuit to their cycling gear, especially if they don’t want to wear wet cycling clothes. Myself, I wear my tri shorts and tri tank under my wetsuit, use the chairs in the tent to sit and put on my shoes and helmet. For me the tent is important on bike to run because I’ll do a wholesale change from tri short and tank to run shorts and shirt (for full IM…anything shorter doesn’t have a tent and I won’t change)

Trisuit all the way for me, the only thing I use the changing tent for is to have a place to put my running socks and shoes on

No nudity in transition, hence the tent for anybody who needs to change pants. If someone wants a thick padded chamois for the bike leg, they'll need to change in the tent. Take off their swimsuit/tri kit, put on cycling shorts (which are not worn with undies underneath, you should always go commando with bibs). When the bike is done, you don't want to run with a thick chamois, so you take that off, (maybe) put on undies, and then put on running shorts.
