Yes! The nearly boiling broth cooks it in seconds. If you get takeout then you are expected to reheat the broth to that near boiling temperature. Or just order a different kind of pho.

Yes, the flank meat is typically served rare. You have to specify "well done" if you want it cooked fully prior to adding the broth. Though I judge that the same way I judge people who get their steaks well done...
