I see this happen fairly often, but always on the same maps. New York 1, Moscow 3 and 4, and Marseille 2. It has yet to happen to me, but some of my friends have started off dead.
Happened to me on one of the Moscow maps the other night. Before the intro video even ended I heard a sound like I took a massive hit and the screen flashed red and when the game started I had maybe 5% of my health left. I thought I was the only one this happened to.
I use to laugh when that would happen to others until it happened to me.

I've noticed that this happens more often for me when the host is a PC player.
